Screening and intake procedures.
From the Academic Intake Assessment, basic info is gathered such as, name​ and schooling information. As well as info that can indicate

the need of special or extra support in some areas such as a language barrier, reading comprehension and/or math skills.

We try to complete them as soon as possible, but have up to 72 hrs to complete assessments.

ORR Intakes places youth in the program and provides relevant information (Special needs, Medical needs, No additional needs)

Youth arrives and begins the intake process at the program. Child is first screened by medical during the intake process. If clinical places the child in an in care safety plan, education will follow in care safety plan as necessary.

How does the facility promote living-unit compatibility based on the characteristics, diverse service needs, and preferences of individuals?

Program provides a safe a welcoming environment to children with diverse needs.

If child is not adjusting, a Treatment Team Meeting will be schedule to accommodate child's needs.

  (FP) What comprehensive assessments do you complete after your initial assessment?

Academic and ESL benchmarks every 6 weeks along with any weekly comprehension tests.

Individual Services Plans (ISP) are completed to evaluate child's strengths and improve their skills.

We establish a positive relationship with the children.

We provide assessments in their language, and make assessments relevant.

We focus on their needs and modify assessments as needed.

(FP) What linguistic and cultural services are provided to the youth during comprehensive assessment to help them engage?

The assessments are translated to their language, if we do not have their language we seek for interpreters.

The iPad is also used to provide translating services.

What is your Special Education Eligibility process for special care needs?

Youth arrives and begins the intake process at the program.

Education conducts a one-time 45-Day screening for youth on the 45th day in care using the Southwest Key 45-Day screening tool.

If no concerns are noted no further action is needed.

If concerns are noted that may require special education or related services the Teacher/ Lead Teacher request Multidisciplinary Evaluation Team (MET) / Treatment Team Meeting is scheduled.

TTM- Follow up on concerns; Does the child meet the “A Child with Disabilities” as defined by IDD

Data Collection

Relevant Information including effects of environment, culture, and economic disadvantage.

If needed a Normed-Reference test by an external party will determine eligibility for special education or related services

Final STEP:

If program can accommodate the youth’s needs, then an Individual Education Plan will be created and implemented.

OR

Services Required beyond program scope, the program will request a transfer facility that can accommodate the youth needs.

  LTFC: How does the program support smooth transition for children into communal living (assess safety issues as applicable)?

Children are placed in an intake class while they receive all the intake assessments, during their intake process, they receive the information needed and a facility tour for a smooth transition. The staff are careful with indicators of previous trauma when initial intakes are completed.

ISP is developed during TTM;

Meeting is documented​

when there is a change in supervision, hospitalization, physical accommodations.

The ISP is updated following state licensing requirements. First 30 days and then every 90 days after initial ISP is done.
