Verification of Employment Hours

• Documentation of the parent's employment that includes the days and hours of employment

o The parent shall obtain and provide documentation of need based on employment that consists of one of the following:

 (1) Pay stubs that indicate the days and hours of employment; or

 (2) Pay stubs that indicate the total hours of employment per pay period, as long as the hours generally correlate with

the parent's requested number of hours needed each week.

• Pay stubs, based on pay period, must be from either month of the two-month window immediately preceding the

certification/ recertification packet due date and not from the current month

OR:

When the pay stubs do not meet the criteria of either subsections ( 1) or (2) above, verify the days and hours, or the total number

of hours of employment each week by doing one of the following:

 Secure an independent written statement from the employer indicating the days and hours of employment (CDA’s

Employment Verification or 3

party verification can be used) using the release of information.

 Telephone the employer to verify the days and hours of employment and maintain a record of the verified information

(CDA’s Verbal Verification can be used).

o If the employer refuses or is non-responsive or employer documentation would adversely affect the parent’s employment in

providing the requested information

• Parent may complete the Employment Need Attestation
