What is a care/support plan?

A care/support plan is a document that allows the individual to detail their day-to-day needs, wishes and preferences. Care plans should be developed by the individual receiving care, however others who know the individual well can be involved where the individual consents, or where the individual lacks capacity to formulate their own plan.
