At the end of Gus Grissom's 1st flight into space, the hatch on his Mercury spacecraft accidentally blew off which caused the capsule to fill with water and sink. The astronaut almost drown as a result. The hatch system was re-designed to prevent similar accidents. While training for his 3rd spaceflight a few years later, a fire broke out inside Grissom's Apollo spacecraft during a launch pad test, that suffocated him and two other astronauts. The hatch redesign triggered by Grissom's 1st spacecraft accident, meant to help save astronauts' lives, is what prevented him from being rescued in the fire accident.
