Yearly Update

• An update on my IEP.

• An update on my special education evaluations.

• An update on my IQ scores.

• An update on my class schedule.

IEP Team

• includes me, my guardian(s), my teacher of record, my teacher(s), my counselor, my dean, an administrator, and any other individuals that support my education

• includes just me and my teacher of record

• includes me, my parents, and my teacher of record

• includes individuals who have requested to be part of my IEP.

Evaluation Date

• This is the date that I completed testing that determined that I could benefit from an IEP.

• This is the date of my last ISTEP testing.

• This is my birthdate.

• This is any annual testing that can be reported on my IEP.

Present Level of Performance

• This is a statement of my skills at the time of my last evaluation.

• This is a statement about me that includes information about what I excel at and what I would like to do after I graduate high school.

• This is a statement about me that states how I am currently doing in school.

• This is part of the IEP that states what testing I will take this year.

Services

• lists supports (like Learning Strategies or speech) in my IEP and how often I receive these supports.

• lists accommodations (extra time on tests) that teachers must legally give me.

• lists permissions (such as a service dog, or sound amplification device) that must be given to me

• lists places (such as IN*Source, Indiana Disabilities Bureau) where my parents can receive extra free support for their children with special needs.

Goals and Objectives

• A goal that I am aiming to complete this year.

• A goal that states what I would like to do after high school.

• A place that notes my progress towards past goals in my IEP.

• A list of skills that I am currently unable to perform.

Accommodations

• example: time and a half on assignments, use of calculator, tests may be read aloud

• example: Learning Strategies, Speech services, Algebra Lab

• example: Transition Services through the Logan Center
