1. List every person who worked in your group.

2. Beside each name rate your group members on a scale of 1 to 5

1: Did not help at all - was non-existent in our group's efforts

2: Helped very little - barely did any of the work and barely assisted with planning

3: Was somewhat helpful - did what was asked but put forth average effort

4: Was very helpful - did their job

5: Extremely helpful - went above and beyond to do great work
