Passive question forms:

(question word) + be + S + V-pp?

Yes/No question:

E.g. The CEO interviewed her.

=> She was interviewed by the CEO.

=> Was she interviewed by the CEO?

E.g. The plumber will fix the water pipe.

=> The water pipe will be fixed by the plumber.

=> Will the water pipe be fixed by the plumber?

Passive question forms:

(question word) + be + S + V-pp?

Wh-H question:

E.g. Cheese is made from milk.

=> What is cheese made from?

E.g. The score will be released tomorrow.

=> When will the score be released?

Does he run his own restaurant?
