-If making “lots of changes,” consider doing a review (annual or other)

-If making changes to the services page, only change the dates of the sections changed. For
example, if I am adding an accommodation, I will only change the date of services for the
accommodation part. This advise does not apply to the cover page.

-Can be done over the phone, in other words, the parent does not need to initial. They still need to
be informed and agree, therefore document their approval within the PR-01.

TGRG: What sections do I amend?

If the team decides to exempt the student from the PROVISIONS of the TGRG then amend:

Section 3: The Profile

Must include data (“Student was administered the Reading Diagnostic Assessment in the Fall of 2022 and was
considered ‘not on track’ to pass the Third Grade Reading Guarantee.”) and discussion (“Student has been receiving
intensive interventions in ______ through his/her RIMP and SDI. Student is currently reading on a __(below 2nd
grade)__ reading level and is increasing approximately 1 grade level of reading every ____ school year(s)). While
student is making progress, it is not expected that he/she will be reading on grade level at the time of the final
assessment, therefore the team decided to exempt student from the provisions of the Third Grade Reading
Guarantee.”

TGRG: What do I write in the amendment PR-01?

Describe the conversation that was had surrounding the decision to exempt the student.

Be sure to include:

-List the team members (that are also required to initial)

-If a formal ‘meeting’ was not held, you should document that the team agreed it was not necessary

-Document attempts to include parent and their response (agree/disagree)
