What is your response if the fire alarm is activated?

• If I heard fire alarm is activated, I will not panic and proceed to the safe place, wait for the BS announcement and fire warden instruction if we need to evacuate the building. I will proceed to the assembly point using the staircase safely, once we reach the assembly point, I will do the head counts and report to the fire warden.

• Run towards the assembly point and inform everyone. Call the emergency response team and proceed to the muster point.

What is your response if you see a real fire?

• Don't do anything. First I will go to assembly point and then inform everyone. I will call security and go to muster point calmly. No running.

• If I saw a real fire, first I will inform everybody using the nearest manual pulling station, and proceed safely our assembly point. Once I reach the assembly point, I will report to the fire warden all the details of the fire incident like the source of fire & the exact location of fire and also report to my team head count.

Reaction during emergencies [others] & injuries, CARE, who to report, how to report

• Leave the space immediately, reach to safe place then raise the alarm. after inform the Security and ICM regarding the incident and wait in muster point.

• In case of emergency like someone was injured. I will make sure proper care management will be given to the injured person. Check the injured person of how severe his injury is and take the personal information for details, if the injury seems like severe do not move him on his position just checked his or her condition and call the emergency ICM number, give the full details of the situation like the condition of the injured person, personal information and location. make sure that the area was secured and safe for the person who l will come for rescue. Do not leave the person and area wait until the assistance come. And go with the patient if needed to bring to the clinic or hospital for medical assistance.

Hazard IDs [what to do, how to report]

• No need to do anything as I have already solved it. Immediately leave the scene and inform to ICM.

• If we observe some hazard we have to make sure that we have all the understanding and knowledge on how to mitigate it or correct it before we put an action. We need to make a report using our HAZARD ID [hazard identification] CARD and submit it into the concerned department or person

Conducting LPO, preparation for LPO

• It is planned suddenly and go with the documents for the LPO.

• Stop work while doing LPO and let the person continue his/her task once LPO is completed.

• We need to create an schedule beforehand to all of high risk task to be observe [target of LPO [Loss Preventive Observation] and assign people to be involve for the observation [observer and observe]

Use of JLA, benefits, importance

• JLA [Job Loss Analysis] serves as reminder of the work place potential work hazard and necessary safeguards to prevent injuries. It addresses potential hazard of the workplace and provides personnel of the clear instruction of the key steps.

• Because of JLA we can we get clearance to complete the job.

• It makes us competent to do the mentioned job in the JLA.

Use of LPSA [3 steps, can articulate], benefits, importance

• ASSESS the Risk- by asking yourself, What could go wrong? What is the worst thing could happen if something goes wrong?

• Address the critical steps of the job or task being performed.

• ACT to ensure the safe operations. Take the necessary action to make sure the job is done safely. Use all the prepared safety standard document like follow the JLA. Seek for assistance if necessary or needed.

• ANALYZE to reduce the Risk; by asking yourself, Do I have all the proper knowledge and training to do the task?, Do I have all the proper tools and personal protective equipment to do the task?,

Responsibilities in Safety

• I am responsible for reporting undesirable behaviors & condition and take disciplinary action. My role is to just report only rest my supervisor will do.

• My main role as shift lead in safety is supervising my team to make sure that they are following all the safety standards ensuring that they are working safely, be with my team onto reinforce them to do the good practice behaviour supporting my team and reinforcing them for their all good practices and guide them properly if they have undesirable behaviour this will leads to our good quality time to work safely and to check them for all of their ideas and concerns.
