How many years is the service commitment of Pharmacy Assistant and RPh?

For Unreconciled Variance:

Amount involved shall be deducted to employee’s account using selling price as reference.

Minor Negligence:

Automatic Dismissal and the Cost of losses shall be charged to employees.

Example:

1.Wrong dispensing of medicine with higher dosage resulting in adverse events and hospitalization of the patient.

2nd offense for violation of company safety requirements or common safety practices involving personal or company property.

3rd Offense for failure or willful disregard to implement proper customer handling (Rudeness, SOP)

Second offense for Tardiness – Lateness in reporting for work

Unused VL's are convertible to cash and will be credited every January and/or February the following year.

•Filing of Vacation Leave must be submitted to the Manager at least One (1) week before the schedule leave.
