The physician shall write an order for patients own medication, the order shall be entered in salamtak with an order command specifying "patients own medication"

If a patient admitted with implanted pumps may be continued, these pumps do not required a safety check by facilities engineering while in use by the patients. Medications shall be prescribed in salamtak with comment administered through patients own implanted pump

Any "patients own medication" is not identifiable by the pharmacist or their condition is of concern will not be used during hospital stay and if the patient insist on its use, what to do?

If the medication is not available in the system, the physician shall accurately enter the details of the medication as "Template Non-Formulary" with the comment "Patient's Own Medication".

The High alert Medication list shall be identified and reviewed regularly (annually at a minimum) through the SEHA Medication Safety Sub-Committee.

Any Medication error that affect the patient or has the potential to do so shall be reported through safety intelligence (SI) by the individual identifying error.
