If the patient received one phase of treatment, you leave phase II Treatment Volume blank.

We only code the first three phases (volume, modality, fraction) under radiation treatment if there are 5 phases.

For “Course Total Dose” we can sum all phases regardless of site.

In the section “Number of Phases” we only code three no more.
