Elaborating all the international rules would be a difficult task. However, we

can simplify the remembering process if we focus on the _______ __________ of these

written documentation of what will occur, or in

some cases, has already occurred, in a research

project. They are designed to provide the

information that is needed to review the project

to ensure compliance with the rules and with

laws and regulations that apply to the project.

and different from previous research. This form must also be accompanied

by the previous year’s abstract and Research Plan/Project Summary. This

should be completed by the Student Researcher.

This form is required for research involving fresh/frozen tissue (including

primary cell lines, human and other primate established cell lines and tissue

cultures), blood, blood products and body fluids. This form also identifies

vertebrate animal tissue will be used in this study. It is to be completed by

the Qualified Scientist / Designated Supervisor.

This form is required for research involving microorganisms, rDNA,

fresh/frozen tissue, blood, blood products and body fluids. It identifies all

potentially hazardous biological agents to be used in this experiment and

describes their method of disposal. This form is to be filled out by a

Qualified Scientist / Designated Supervisor in collaboration with the student

researcher(s) and completed by a local Scientific Review Committee.

participants not at a Regulated Research Institution. If at a Regulated

Research Institution, researchers should use institutional approval forms for

documentation of prior review and approval.

microorganisms, or devices that will be used in the research project. It also

identifies the risks and hazards involved in the process. This form must be

completed by the student researcher (in collaboration with Qualified

Scientist/Designated Supervisor) before experimentation.

This document includes the rationale, research questions, procedures, data

analysis and others. It also contains some subject-specific guidelines that

are applied in the research project. This document is usually attached to the

Student Checklist (1A).

understand the risks and possible dangers involved in the Research

Plan/Project Summary. This form is to be filled out by the students,

parents, and completed by a local Scientific Review Committee. It is required

for each student, including all team members.

the student research conducted in a Regulated Research Institution,

industrial setting, or any work site other than home, school, or field.

Scientist reviewed and approved the Research Plan/ Project Summary prior

to the start of the experimentation. It must be completed and signed by a
