Three ways to ask one person
"How are you?"
¿Cómo está usted? (notice the formal "you")
¿Cómo estás tú? (notice the informal "you")
¿Qué tal? (This one is slang. Use it in informal settings.)

Remember that "formal vs. informal" shows up in which "you" is used.
tú is "you" when speaking to someone familiar to you. Friends, family, anyone in your peer group. Or a child in a younger age group than you.
usted is "you" when speaking to someone you haven't met before, or anyone outside your family who is your elder. Use usted when the relationship is a professional one.

Unless you are given this invitation, use the formal usted.

You can leave out the "tú", because the verb "estás" makes it clear.

You should not leave out usted.
Subject pronouns (Yo, tú, usted, él, ella, nosotros, ustedes, ellos, ellas) are often left out is Spanish sentences, because the verb form or other context make the subject clear.
However, "usted" not left out as often. Keep it in, as an extra measure of respect.
