Donatella recently came out of retirement and began working at Walmart to help pay her rent which has increased to the point where she doesn't know if she'll be able to afford living there. "I've lived in this apartment for almost 10-years. The rent has never gone up hundreds off dollars like this. You have been working with Raphael for about 6-months after he was diagnosed with hypertension. Since then, you have been working with him on the importance of taking his high blood pressure medication and incorporating exercise. He is always pleasant, cheerful, and joking with staff. However, he has not made much progress in changing behaviors. He says that his entire family is overweight and has high blood pressure and diabetes so it's probably in the cards for him anyways.

At his most recent doctor's appointment, Raphael was diagnosed with Type 2 diabetes and prescribed an injectable insulin.

At your follow-up appointment, Raphael brings his wife and 4-year old daughter. "There's my favorite person!" Rafael says to you. His wife looks concerned: "I wanted to come with him today because I'm scared for my husband. He's just not taking this as seriously as I am." He's not even taking the insulin. I'd give it to him if I need how."

Raphael says jokingly, "There's no way I'm letting you use that needle on me! I'll be fine! I just don't have time to exercise ALL the time. And you know we can't afford a gym membership. And who will watch our daughter? Also I'm a big guy. I need more to eat than salad and fruit. My entire family has had diabetes and high blood pressure." His wife looks at you and says, "Exactly! His father died from a heart attack and his mother had to have her foot amputated." She turns to Raphael and says, you're only 10 years away from the age your father died. What about our daughter!" Raphael's smile fades. He sits down and says, "Ok, let's talk about my action plan. Let me try harder."
