Impingement syndrome (swimmer's shoulder) - Impingement is the pinching of soft tissue, such as a bursa, tendon, or nerve, between the ends of two or more bones. The joint most susceptible to impingement is the subacromial space in the glenohumeral joint of the shoulder. The supraspinatus gets pinched, causing pain. May be caused by repeated overhead activities like throwing, swimming, swinging in tennis and serving a volleyball. An impingement of the subacromial bursa will create symptoms of weakness in the supraspinatus muscle and point tenderness of the glenohumeral joint when the arm is elevated. The Hawkins-Kennedy test for this impingement uses compression of the supraspinatus tendon in the subacromion space to access the possibility of impingement of the subacromial bursa - it will be painful. The winged scapula test will test the long thoracic nerve for impingement. Impingement of the nerves in the elbow and wrist can be tested with the Tinel's sign which will produce a tingling sensation by percussion of the ulnar nerve and carpal tunnel.
